Position: Primary Care Physician

Pay:& $260,000.00 - $312,000.00 per year

Location: Jacksonville, Florida, Pennsylvania, North Carolina, Arizona, Ohio, Wisconsin, Missouri, Arkansas

Position& Type:& Full-Time

Shift:& Monday-Friday 8:00 AM – 5:00 PM

Compensation:

Base Salary: $260,000

Bonus: Up to 20% of base salary based on:

Patient satisfaction ratings

Alignment with organizational values

HEDIS compliance metrics

Reduction of hospital sick days

Model: Value-Based Care (Outpatient)

Patient Population: Seniors with 3–5 chronic conditions (e.g., heart disease, diabetes, hypertension)

Patient Load: 10–12 patients per day

Panel Size: Maximum of 650

Schedule: Monday–Friday, 8:00 AM – 5:00 PM

Call Schedule: Based on market provider count (e.g., 1:32 weeks if 32 physicians in region)

Care Team Support: Each physician is partnered with a Medical Assistant, Scheduler, Referral Specialist, and full administrative and leadership support

Requirements:

Doctor of Medicine (MD) or Doctor of Osteopathy (DO)

Board Certification in Family Medicine or Internal Medicine (Required)

Fellowship in Geriatrics (Preferred but not required)

Current, unrestricted medical license in the applicable state

Current DEA registration

Strong proficiency in office-based procedures, including:

Suturing

Joint injections

Skin biopsies

Basic wound care

Excellent communication, teamwork, and patient relationship skills
