About the Role

We are seeking an experienced and compassionate Pediatric Care Manager to join our healthcare team. The ideal candidate will be responsible for coordinating and managing care for pediatric patients to ensure quality outcomes and holistic family support. This role involves collaboration with physicians, families, and community partners, requiring both clinical expertise and strong interpersonal skills.

Key Responsibilities

Manage and coordinate care plans for pediatric patients across various settings.

Conduct assessments and provide guidance to families to ensure comprehensive care delivery.

Collaborate with physicians, behavioural health professionals, and community resources.

Monitor progress, track outcomes, and adjust care plans, as necessary.

Conduct home visits as needed and ensure continuity of care.

Maintain accurate and timely documentation in compliance with regulatory standards.

Qualifications

Active LCSW, LMHC, or LPC license (required).

2–4 years of pediatric experience in a clinical, community health, or behavioural health setting.

Must reside in one of the following locations: Boca Raton, Boynton Beach, Delray Beach, or West Palm Beach, FL.

Experience with home visits preferred.

Bilingual (Spanish English) candidates strongly encouraged to apply.

Master’s degree in behavioural health, Counselling, or Social Work required.

Additional Information

Hybrid position with approximately 50% travel.
