We are currently seeking an experienced: HOSPICE & PALLIATIVE CARE NURSE PRACTITIONER Full Time (Covering Bladen, Columbus, Robeson) Job Description:Implements palliative care program requirementsCompletes medical visit with patients per guidelines and coordinate care with Palliative Care team as clinically indicatedConduct face to face encounter to provide re-certification information to Medical Director/Associate Medical Director for Hospice PatientsProvide clinical findings of that face to face encounter Primary care physician, and/or supervising Medical Director/Associate Medical DirectorCommunicate with patients/families at all levels.Communicate with all agency personnel Job Requirements:Master’s degree in Nursing required, with current licensure in the state of practiceCertification as an Adult or Family Nurse PractitionerExperience in Hospice or Palliative Care nursing preferredKnowledge of the framework, function, and organization of a hospice care program.Excellent communication and organization skills.
