As a Health & Wellbeing Practitioner, you’ll be part of a multidisciplinary team delivering recovery-orientated services to women aged 18+. Working on-site at HMP Downview, Sutton, your role will involve: Managing a caseload with a person-centred approach Conducting assessments and co-producing care plans Delivering structured 1:1 sessions and facilitating group work Providing harm reduction advice and support Performing second signatory duties Supporting release planning and community reintegration Accurately recording data and outcomes to measure success
