You'll work closely with patients, physicians, and other healthcare professionals to deliver comprehensive support and improve health outcomes from the comfort of your home.

Patient Educator :

You review proper technique (blood pressure, pulse,

checking glucose level, etc.)

Assess educational needs and health literacy

Test device and technique with patients

Set expectations with patients on when/how to

escalate symptoms and concerns related to device readings.

Requirements

Certiifed LPN or relevant healthcare degree or certification.

Possess strong communication and interpersonal skills in English.

Proficient in using electronic health records and care management software.

experience in at least one of the following settings: remote patient monitoring program, disease management, home health care, hospital, or physician office setting

Knowledge of chronic care monitoring – preferred

Strong computer skills

Outgoing and friendly personality
