Job Description
Provide social work services to patients and families to assist them in continuity of care planning and in enhancing their capability of coping with illness and daily functioning. Assist the individual and family with psycho social needs which may compromise adaptation.
Essential Duties & Responsibilities:
Interviews patients and family members.
Identifies psychosocial and environmental needs and resources
Participate in conferences on behalf of patients.
Evaluates data, and plans pertinent therapy programs for patients.
Provides information and referrals for insurance and entitlements benefits
Plans and arranges temporary and permanent changes in living arrangements.
Helps patients and their families through individual or group conferences.
Follows medical recommendations and provides services planned to restore patients to optimal social and health adjustments.
Advises on social problems, arranges for discharge and post-operative care, assists in placement of adults in nursing homes.
Utilizes community agencies and resources.
Prepares and keeps current a case record for all patients being followed.
Make summaries of information.
Participates in educational programs for personal development.
Actively participates in Interdisciplinary treatment team meetings, treatment planning and conference.
Participates in treatment team meetings and documents follow-up necessary to update and keep treatment plan current
Provides group and individual sessions as ordered and in accordance with client needs, focusing on individualized needs of each client.
Attends staffing with the program director to ensure continuum of care.
Education: Master's degree in Social Work, Counseling, Psychology or related field required.
License/Certification: Licensure as LCSW or LPC or equivalent in the State of Louisiana required. CPR required.
Additional experience:
Experience in inpatient psychiatric or behavioral health setting preferred.
Excellent communication and interpersonal skills.
Ability to work effectively in a fast-paced multidisciplinary team environment.
Familiarity with electronic medical records systems.
