接待 通知和陪伴长者及其家属Receive, inform and accompany the residents and their families
1) 接待并安置新入住长者Receive and arrange the new admitted residents.
2) 注意倾听和交流 保证长者身心健康 生活舒适 Pay attention to listen to and communicate, ensure the residents’ physical and mental health, and the comfortable life.
3) 陪护长者及其家属Accompany the residents and their families.
4) 及时回应长者的请求 如呼叫器 Respond to the request of the residents in a timely manner (such as the call device).
5) 严格按照机构各项规章制度和技术操作规范为长者服务Keep strict accordance with the facility’s rules and regulations and technical operation norms to serve the residents.
6) 按照整合性照护计划和养老机构活动计划 做好长者陪护工作 In accordance with the integrated care plan and facility’s activity plan, accompany the residents well.
7) 组织娱乐活动Organize recreational activities.

10) 协助长者行走 转移 保证长者转移后安置舒适 防止跌倒 尽量鼓励长者自主完成 保持长者的自主活动能力 如出现跌倒的情况 按照有关的规程处理 要求过程可追溯 Help the residents to walk, transfer, ensure the residents’ comfort after the transfer, prevent falls, encourage the residents to complete autonomously as far as possible, to maintain the independent activity ability of the residents. In case of falls, treat according to the relevant procedures (it’s required the process can be traced back).
11) 为长者提供卫生 舒适照护 梳洗 泡澡 冲澡 洗头 洗脚 擦身等 预防压疮 做好口腔照护 确保假牙 助听器及自主活动器械的安装 佩戴和维护 做好皮肤和粘膜的保护 如出现异常症状 压疮 皮疹 瘙痒 伤口等 及时向护士汇报 Provide health and comfort care for the residents (grooming, bath, shower, wash hair and foot, brush, etc.) to prevent pressure sores. Do a good job of oral nursing care. Ensure the dentures, hearing aid and independent activity instruments’ installation, wearing and maintenance. Protect well the skin and mucous membranes, such as abnormal symptoms (pressure ulcers, skin rash, itching, wounds, etc.), should report to the nurses in a timely manner.
12) 为长者提供饮食相关的照护 参与饭菜发放并协助长者进食 预防营养不良和脱水现象 确保长者的特殊饮食计划得到遵守 严格遵守长者个人生活计划 按照规定的食物摄入量 进餐频率和时间以及饮食要求 少盐 不吃糖… 协助长者进餐 及时发现并汇报长者在饮食方面的问题 如厌食 呛咳 误吸 呕吐 吞咽困难等 及时记录监控表 定期为长者称重并记录饮食监控表 Provide food related care for the residents: participate in the food distribution and assist the residents to eat, prevent malnutrition and dehydration phenomenon, ensure the residents' special diet plans have been observed. Strictly abide by the residents’ personal life plan, assist the residents for meals in accordance with the provisions of the food intake, eating frequency and time and diet requirement (less salt, don't eat sugar...). Find and report the residents’ diet related problems in a timely manner (such as anorexia, choking cough, aspiration, vomiting, swallowing difficulty, etc.), make record in the monitoring form. Weigh regularly for the residents and make record in the diet monitoring form.
13) 在充 尊重长者尊严的前提下 做好长者大小便照护 及时更换尿裤并清洁会阴部 密切观察长者的大小便情况并准确记录 保证其可追溯 一旦发现异常及时汇报 按照排便计划 帮助长者如厕或使用便盆 预防便秘 Under the premise of full respect for the dignity of the residents, do a good job of defecation nursing for the residents (in a timely manner to change the diaper and clean the genitals). Keep close observation of the reisdents’ defecation and make correct record, ensure that it can be traced back, once found abnormal situation, should report it timely. In accordance with the defecation plan, help the residents to go to toilet, or use the toilet bedpan, prevent the constipation.
14) 常规体征监测 体温 脉搏等 Routine physical sign monitoring (body temperature, pulse, etc.).
15) 严格遵守安全规范 一旦发现长者有危险行为或长者健康状况发生任何变化时 应及时向护士汇报 Keep strict compliance with the safety rules, if find the residents have any risk of behavior or change of health status, should report to the nurses timely.
在照护主管的指导下 帮助护士完成护理操作和其他工作Under the guidance of the chief nurse, help the nurses to complete the nursing operation and other work
16) 如出现紧急情况 采取必要的急救措施If there is an emergency, take the necessary emergency measures.
17) 如长者需包扎 体检或取样 须完成相关准备工作 安置好长者If the residents need to bind up, do physical examination or do sampling, should complete the relevant preparatory work, and arrange the residents well.
18) 在护士的指导下 做好简单的包扎操作Under the guidance of the nurse, do a simple dressing operation.
19) 在护士分发药物后 协助长者服药Assist the residents to take medicine after the nurses have distributed the medicine.
20) 帮长者穿压力袜Help the residents to wear pressure socks.
21) 如果长者过世 应完成必要的卫生照护和清洁消毒工作 If the resident died, the necessary health care and cleaning and disinfection work should be done.
居住环境和设备的维护Maintenance of living environment and equipment
22) 维护长者生活环境 确保环境干净 整洁 舒适 安全 Keep the maintenance of the residents’ living environment, ensure that the environment is clean, tidy, comfortable, and safe.
23) 铺床换床单 及时整理床铺 定期或必要时进行消毒 以保持床铺的清洁卫生 Change the bed linen and make the bed in a timely manner, disinfect it regularly or when necessary to keep the bed’s clean and sanitary.
24) 按照相关规定和流程对废物进行处理 且管理好衣物 毛巾 床上用品等流通过程 Treat the waste in accordance with the relevant provisions and procedures, and manage well the clothing, towels, bedding, and other circulation process.
25) 做好生活辅具的清洁和消毒Do a good job of life assistive equipment’s cleaning and disinfection.
26) 检查医疗设备和器材的功能及运转状况 如有异常及时汇报Check the function and operation status of medical equipment, if there is any abnormal situation, should report timely.

29) 参加多学科的例会 参与长者个人护理计划的制定 Participate in the regular meetings of multidiscipline team, participate in the development of personal care plan for the resident.

30) 如有需要 接听电话If necessary, answer the call.
31) 特别关注机构内安全隐患 如火灾隐患等 Pay special attention to the internal security risks, such as fire hazards, etc.
