Salary: $50,000
Upper Valley Services (UVS) is one of Vermonts designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.
UVS is seeking a Service Coordinator candidate who is:
Detail orientated
Familiar with documentation
Values teamwork but works well independently too
Experience with people with developmental disabilities
Works well with deadlines

We are offering a generous sign on bonus! $500 at time of hire and another $500 upon successfully completing six months at UVS
Upper Valley Services is a person-centered program.
Service Coordinators meet at least monthly with the shared living provider and participant in their home to support and manage various aspects of the state-required expectations to ensure a healthy environment that meets the participants goals and needs.
Mix of field work and main office in Moretown.
Background checks are required
Must have valid drivers license and insured vehicle

M-F 8am-4pm and occasional evening or weekend availability
College degree in Human Services or related field preferred.
