Job Summary:

- Assess patients to determine mental, physical, and spiritual needs

- Develop, implement, and evaluate individualized care plans in collaboration with a multidisciplinary healthcare team

- Provide explanations and instructions to patients and/or their responsible persons

- Utilize nursing clinical judgment to recognize and report changes in patient condition

- Contribute to a healthy work environment by managing conflict and using professional communication skills

- Fulfill Core Measure responsibilities related to patient care

- Participate in continuing education to maintain and enhance professional skills

- Maintain complete and accurate medical records

- Hold a current Illinois Registered Nurse license

- Maintain current CPR/BLS certification
