Shift: Sunday–Thursday 8:00 AM – 4:00 PM
Some on-call required

Guaranteed Hours: 37/week
Contract Length: 13 weeks (91 days)

Key Responsibilities:
1. Clinical & Case Management Duties
Acts as case manager for assigned patients
Performs OASIS documentation accurately and timely
Utilizes evidence-based nursing process for assessments, interventions, and evaluations
Creates and manages realistic plans of care, including patient and caregiver input
Maintains and reviews accurate nursing documentation
2. Skills & Equipment
Must be comfortable with high-tech skills:
IV therapy, wound VAC, complex wound care, infection control
Familiar with McKesson processes (e.g., visit lock-down, follow-ups)
3. Interdisciplinary Coordination
Coordinates care with physicians, providers, therapists, social workers, etc.
Supervises LPNs and HCAs, delegating tasks appropriately
Identifies additional service needs (PT, OT, SW, etc.)
Facilitates safe discharges and transfers
4. Compliance & Education
Ensures all documentation follows State, Federal, and agency policies
Educates patients and caregivers (medications, discharge instructions, safety concerns, etc.)
Completes all forms like HHABN, Medicare Notice of Non-Coverage in a timely manner
5. Administrative & Team Responsibilities
Communicates with schedulers to avoid conflicts in visit types
Participates in monthly peer reviews and provides/receives feedback professionally
Proactively leads case conferences with team members and managers
Advocates for high-quality, patient-sensitive care
Qualifications Required:
Active Vermont RN License (in good standing)
Strong knowledge and hands-on experience with OASIS documentation
Experience in home health nursing, case management, and patient education
Up-to-date knowledge of state and federal home health regulations
Ability to work independently and manage a caseload
