Responsibilities: - Evaluate and assess patients' physical conditions and develop individualized treatment plans.

- Implement therapeutic exercises and techniques to relieve pain and improve mobility, strength, and coordination.

- Educate patients and caregivers on exercises, safety precautions, and self-care strategies.

- Collaborate with an interdisciplinary team to provide holistic, patient-centered care.

- Maintain accurate and timely documentation of patient progress and treatment plans.

Qualifications: - Active Physical Therapist license in Oregon, or the ability to obtain.

- Master or Doctorate degree in Physical Therapy from a CAPT accredited school.

- Minimum of 1 year of experience as a Physical Therapist.

- Home health experience is strongly preferred.

- Fluent in English.

- Current BLS, hands-on CPR certification.

- Reliable transportation with a valid driver’s license and auto insurance.

* If education is from a non-CAPT accredited school, an evaluation by a credentialing agency, such as FCCPT, is required.
