Education and Experience:
Must have bachelor's degree in social, behavior or human services field from an accredited university
Must be able to qualify as a Qualified Mental Health Professional (QMHP)
Must have valid TX driver's license and be insurable
Job Purpose:
Ensures a person is provided uninterrupted services during a transition between inpatient and outpatient services
Assists with admissions, discharges, and tracking of significant events
Participating in developing and reviewing a person's recovery or treatment plan
Job Function:
Work with Intake LPC and treatment team to provide any valuable information for on-going assessments
Maintain contact with hospital staff for all consumers in the hospital
Coordinate discharge and follow-up services including doctor appointments
Provide crisis intervention services as needed
