Resident Service Coordinator

Location: Guardian Angels Housing - Elk River & Zimmerman Locations

Salary Range: $44,000 - $60,000/Year (Based on Experience)

Position Summary:

The Resident Service Coordinator meets with seniors in assigned building as needed to ensure their individual needs are met and connect them to community resources and programs as appropriate. This role advocates for seniors and meets with individuals, families, or outside agencies as appropriate. Build and develop relationships with community resources.

Schedule:

Full Time, Monday - Friday.

Zimmerman Location - Office Hours Weekly on Thursday & Friday Responsibilities:

Provides advocacy, information, and referrals to residents throughout Guardian Angels' independent apartment buildings.

Communicates pertinent issues involving tenants' inability to live independently to family members, the Director of Housing, and appropriate members of the Guardian Angels team.

Maintains close relationships with community programs and all Guardian Angels programs available to meet the needs of tenants. Bring resources into the building when possible.

Maintains up-to-date knowledge of state assistance programs and understands Medicare and supplemental insurance programs.

Collects information on services and resources available to seniors within Sherburne County.

Develop collaborative efforts for families, friends, and volunteers to create a support network for aging tenants.

Advocate for residents in resolving problems ranging from medical bills and insurance claims to financial and forms assistance.

Communicate changes in tenants' behavior to designated representatives, suggest appropriate services and schedule, and lead family care conferences if needed.

Communicate with hospital and nursing home social workers to assist in the coordination of discharge planning when residents are returning from the hospital or nursing home.

Locate and attend professional meetings, conferences, and trainings to keep up with current practices, programs, and policies regarding services for seniors.

Participates in trainings, in-services, and other meetings to be compliant with HUD and management requirements.

Completes move-in questionnaire and orientation with new tenants within 2 months of move-in.

Maintains appropriate documentation for HUD quarterly reporting and 504 and Accessible Housing Services.

Coordinates NAPS program for Elk River, Becker, and Zimmerman area.

Staff office hours in Zimmerman buildings on Thursdays and Fridays or as needed.

Follows universal precautions, facility infection control guidelines, sanitation, and safety standards of practice in all work and activities.

All duties will be performed in accordance with current standards, guidelines, state, and federal regulations.

Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA.

Uphold the mission, vision, and values of Guardian Angels.

Performs all other duties as assigned.

Qualifications:

Bachelor's Degree in Social Work/Psychology or related field required.

Two to three years of previous experience working with elderly and disability services, knowledge of referral processes, and demonstrated working knowledge of local services.

Knowledgeable of state assistance programs and basic knowledge of Medicare and supplemental insurance programs.

Knowledge of resources available to seniors, including state assistance programs.

Ability to problem solve and handle difficult or stressful situations.

Advanced computer skills, including Microsoft Office 365. Proficiency in Excel and database management is required.

Excellent verbal and written (read, write) communication skills.

Self-motivated, ability to multitask and prioritize. Attention to detail is a must.

Maintains good attendance and punctuality.

Ability to work occasional evenings and weekends to assist with events.

Ability to travel to different buildings as required to meet with tenants.
