About the Role

As a Nurse Practitioner at Matrix, your role involves conducting thorough health assessments during visits that can last up to one hour. You will evaluate a member's health status, medical history, medication adherence, and social environment, providing insights that are hard to achieve in standard office visits. Your responsibilities include:

Performing adult and geriatric assessments including medical history reviews, diagnoses, treatment plans, health education, physician referrals, and case management referrals.

Administering and ordering necessary point of care and lab-based tests.

Collaborating with physicians, case managers, social workers, family members, and other healthcare personnel.

Working closely with Primary Care Physicians to enhance patient education and ensure follow-up care.

Providing services at members' residences, including their homes or Skilled Nursing Facilities.

Qualifications:

A Master's Degree or equivalent experience along with NP licensure.

Current NP licensure with prescription authority in your state or the ability to obtain it.

Board certification from AANP, AACN, or ANCC in relevant nursing specialties.

Current BLS, ACLS, or CPR certification.

At least one year of NP experience preferred, but new graduates are encouraged to apply!

A valid state driver's license and proof of adequate auto insurance.

Strong computer skills and experience with health record software.

Excellent verbal and written communication skills.
