Position: School Based Outpatient Therapist
Reports to: Clinical Director
Status: Full-Time
Responsibilities and Duties:
Provide and document therapeutic services within our scope of practice, including accurate psycho-social and diagnostic assessment, treatment planning, and individual and family counseling according to best practice standards or generally accepted industry practices.
Maintain a clinical record of therapeutic encounters (i.e. progress notes, record of service, treatment plans, discharge plans, etc…)
Ensure that progress notes are completed within 48 hours of service to ensure accuracy of documentation and timely billing for services.
Participate in treatment team and staff meetings as required.
Provide consultation with other service providers as needed with timely and accurate documentation of any referrals or communication with outside providers.
Provide and document appropriate discharge planning, aftercare planning, and referrals to all clients, document appropriately, and obtain required patient signatures.
Provide accurate availability to administrative staff for scheduling purposes.
Attend staff meetings as scheduled or otherwise obtain relevant administrative information and updates. It is the therapist’s responsibility to be aware of all policies and procedures, including updates and revisions, as they are made available.
Maintain and abide by all supervisory agreements either internal or external to the organization as required in addition to those required for professional licensure and/or certification.
Review and abide by all relevant policies and procedures and published updates or memorandums.
Provide critical incident reporting and mandated child abuse reporting as required by law to the COO as appropriate within 24 hours of incident or incident notification.
Maintain consistent communication with the family (at least one time per month) during the child’s session.
Must follow the school dress code at all times.
Maintain accurate encounter logs and send home for signatures every 3 months.
Follow a regular, set, weekly schedule, and assure that the school based liaison has your latest schedule.
Obtain monthly supervisions and keep a regular supervision log.
Follow all HIPAA guidelines, especially in regard to school communication.

Minimum Qualifications and Competency Requirements:
Possess a degree in a mental health related field AND
Be eligible for professional licensure as an LPC, LSW, LCSW, LMFT, or Psychologist OR
Possess a professional license as an LPC, LSW, LCSW, LMFT, or Psychologist AND
Possess state criminal, child abuse, and FBI clearances without a prohibition to work with children or elderly patients AND
Have taken the mandated reporter training within the past 5 years.
