SUMMARY OF FUNCTION: To provide permanency planning and direct emotional support and technical assistance to the families involved in the adolescents care and treatment. The Transitional Case Planner supports the birth parents perseverance with children through challenging periods of extreme and troubling behavior and models appropriate responses and teaching strategies. Guiding adolescent birth parents and foster parents through a proactive process of assessment, goal setting, and treatment planning to address the childrens current emotional/behavioral problems, accomplishes this task. specific treatment plans. Case Planner use the SBC model (Solution Based Casework) as a conceptual framework for working with families, youth and their connections.
MAJOR DUTIES AND RESPONSIBILITIES
To establish a plan to safely bring adolescents and young adults on AWOC status back to program and/or discharge them to a safe and stable discharge resource.
Maintain all required contacts with the adolescent and potential discharge
resource/s.
Develops treatment and permanency plans for the youth and family through a collaborative process involving all key stakeholders and assists them in the implementation of the treatment plan for the adolescent.
Attends regularly scheduled Family Court Permanency Hearings and prepares Permanency Reports in advance of hearings.
Coordinates respite and crisis therapeutic services in the program.
Collaborates in the agencys establishment and development of linkages with relevant community services. Attends meetings with service providers to monitor the youths and familys use of recommended services.
Ensures youths participation in youth development (i.e., educational, vocational, money management, housing, future-planning, etc.) workshops.
Documents all activities related to the child and family, as required by the agency and ACS policy and procedures.
Participates in all case planning conferences.
Maintains visits with child, birth parents, and siblings.
Responsible for providing emergency case coverage as needed.
Provides timely, accurate and detailed documentation to support casework services, including but not limited to Progress Notes, Plan Amendments, FASPs, Permanency Reports, etc.
13. Provides referrals to child/family as needed (i.e.: chemical abuse, medical or
mental health, recreational) from a community-based perspective.
QUALIFICATIONS:
BA/BSW required.
Experience working with adolescents and a passionate commitment to permanency for adolescents
Good writing and word processing skills. Proven ability to meet deadlines and other mandatory documentation and contact requirements
Ability to work a flexible schedule, including periodic evenings and weekends, ability to travel to community settings throughout NYC
NYS Drivers license preferred
