Description:
This full-time position will be responsible for providing end-of-life care and hospice services to patients in their home or community setting. The Licensed Practical Nurse (LPN) is responsible for assisting in planning for the patients and familys care under the direction of the RN- Case Manager. The LPN will provide skilled nursing care as directed by the RN Case Manager and as described in the interdisciplinary plan of care and will reinforce appropriate care techniques for patients and families. The LPN in this position will work closely with the RN- Case Manager to develop rapport with the communities and facilities where our patients reside. This position will report to our Kennesaw office and will cover the area between Kennesaw and Cartersville.
Schedule:
Monday, Tuesday, Thursday, Friday
Saturday and Sunday every other week
Every Wednesday and every other Saturday and Sunday off
Requirements:
Strong ability to specifically develop and cater to/work with staff members of communities and facilities.
Current Licensed Practical Nurse License.
A self-driven individual with a desire to deliver excellent customer service.
Excellent communication skills, verbal and written.
Excellent clinical assessment skills.
Computer skills (EMT, email, and familiarity with health care applications).
Excellent time management skills.
Ability to accurately document a patient's current status.
Knowledge of pain management & symptom control.
Valid Driver's License and reliable transportation
The ideal candidate will live in or around the Kennesaw or Cartersville area.
