CLINICAL NURSE SPECIALIST / DISCHARGE PLANNER

Part Time Position - 30.4 Hours per week

An exciting opportunity exists for a Discharge Planner to join our dynamic team at Hunter Valley Private Hospital.

We are seeking to employ a Discharge Planner to manage our Inpatient Rehabilitation Programs on a permanent part-time basis.

The Discharge Planner will have an evidence based clinical approach to quality and the patient care framework and work with our hospital teams to co-ordinate the discharge of all Rehabilitation patients. The successful applicant will have evidenced experience in this role or similar.

As Discharge Planner your responsibilities will include:

Evidenced experience co-ordinating and planning appropriate and effective discharge of patients, either to their usual accommodation or to suitable accommodation as discussed and implemented through discussion with the patient, their families, and the multi-disciplinary teams and community services.

Evidenced professional portfolio development with a commitment to ongoing learning and development.

Hunter Valley Private Hospital is located on the fringe of Newcastle, close to beaches and the Hunter Valley and is close to the Pacific Motorway, vineyards and university. The hospital is an 83 bed facility which offers a variety of surgical, medical and rehabilitation services.

election Criteria:

Bachelor’s Degree in Nursing

Full registration with AHPRA

At least 5 years’ experience as a senior acute care nurse

Proven ability to implement programs and identify gaps within the requirements of the role

Excellent customer service and interpersonal skills

Demonstrated communication skills

Excellent IT skills

Excellent communication and negotiation skills
