Position Overview:

We are seeking a Hospice RN Case Manager to join our team of dedicated healthcare professionals. The RN Case Manager plays a vital role in coordinating care, ensuring quality services, and supporting patients and families through one of life’s most meaningful journeys.

Key Responsibilities:

Provide comprehensive, compassionate nursing care to hospice patients in their homes, facilities, or other care settings.

Develop individualized care plans in collaboration with physicians, social workers, and the interdisciplinary team.

Serve as the primary point of contact for patients and families, ensuring clear communication and emotional support.

Monitor and manage symptoms, administer medications, and provide education on end-of-life care.

Maintain accurate and timely clinical documentation.

Participate in on-call rotation as needed to provide 24/7 support for patients and families.

Qualifications:

Current RN license in the state of Pennsylvania.

Minimum of 1–2 years of nursing experience (hospice or home health experience preferred).

Strong communication, organizational, and critical-thinking skills.

Compassionate, patient-centered approach to care.

Valid driver’s license and reliable transportation.
