Qualifications REQUIRED QUALIFICATIONSPhysician Licensed to practice medicine in current state.Licensed by the Federal Drug Enforcement Administration and the State of Illinois to prescribe narcotics, if applicable.Board certified or board eligible in the appropriate specialty(ies).
