Job Responsibilities: Complex Clinical Care
Manage patients with multiple chronic and complex conditions using a team-based approach.
Collaborate with social work, case management, behavioral health, pharmacy, and nutrition to develop and implement care plans.
Lead transitional care planning across the continuumfrom hospital to home.
Coordinate with hospitals, ERs, and skilled nursing facilities to ensure safe and timely discharges.
Treat acute exacerbations onsite (e.g., CHF, COPD, infections) and determine when escalation is necessary.
Accept urgent/same-day visits post-discharge to prevent readmissions.
Monitor disease progression and proactively prevent ER visits and hospitalizations.
Educate patients on chronic disease management and advanced care planning.
Practice evidence-based medicine aligned with the PCMH model.
Qualifications: Required:
MD or DO with active, unrestricted license in Ohio
Board Certified in Internal Medicine or Family Medicine
BLS certification and DEA license
EMR proficiency and comfort with technology
Passion for serving underserved populations
Collaborative, team-oriented mindset
Preferred:
Experience in population health or value-based care
Background working with medically and psycho-socially complex patients
Working Hours: Monday-Friday 8-5
