Nurse/Care Manager Level 2 – Boys Community Residence GRADE 13 $62,608-$76,107.20 (Exempt) FLSA

I. QUALIFICATIONS

Education/Experience

NYS Registered Nurse License, BSN preferred

Two years’ professional nursing experience

One year experience with DD/MH population preferred

Skills

Interpersonal, organizational and communication skills

Ability to instruct staff and individuals in basic health care

Ability to work with the Interdisciplinary Treatment Team

Familiar with community agencies and resources

Familiar with computer programs (e.g. Word, Excel, EHR)

Ability to legally operate a vehicle in NYS

The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighting up to 40 lbs.

II. RESPONSIBLE TO: Supervising Nurse/Care Manager

III. MAJOR FUNCTIONS

Primary Duties

Work as part of a Care Coordination team

Assist/cover for other nursing staff absences

Provide medication education to staff and individuals

Participate in multidisciplinary team meetings and case reviews

Identify and advocate for individual rights

Teach/assist in developing/maintaining ADL skills

Teach medication administration/supervision to staff

Certify DD staff to pass medications

Conduct initial and ongoing assessments

Monitor routine health care of all individuals

Provide/arrange for routine and emergency healthcare

Review all health-related incidents

Oversee/order all medications

Maintain health records according to standards and regulations

Provide in-service training regarding health issues

Maintain/coordinate effective and on-going communication with other service providers

Participate in the development/documentation/review and update of care plan

Act as linkage to community services

Coordinate and conduct care planning activities for a designated case load of recipients

May utilize health information technology to coordinate care and communicate with providers

Act as medical resource consultant for team members

Promote health and wellness activities for recipients via individual or group sessions

Provide crisis intervention services as necessary

Provide transportation as needed to necessary services

Promote self-advocacy and personal empowerment

May be assigned to participate in rotating afterhours/weekend on call coverage

Establish relationship with all individuals, family members, and advocates

Provide blood borne pathogens and other training to program and agency staff as needed

Understand and follow all program policies and procedures

Represent the agency and program in the community and with other service providers

Participate in and successfully complete all required staff training and development activities

Regular and substantial contact with individuals receiving services

Perform other related tasks as directed by supervisor
