What You'll Do:

Travel daily to assigned MN service areas to visit clients in their private homes

Conduct functional and cognitive evaluations to develop tailored therapy plans aligned with Medicaid and state guidelines

Administer restorative and maintenance treatments; supervise OTA contributions and ensure compliance with billing regulations

Provide guidance on home modifications, assistive devices, and caregiver training to enhance functional outcomes

Maintain accurate records of services and collaborate with care teams to align therapy with broader treatment goals

What You'll Need:

Active MN Occupational Therapy License

Graduate of an accredited OT program

CPR certification

Reliable vehicle with valid license and insurance

Home care or geriatric experience is preferred
