We are offering a generous sign on bonus! $500 after successfully completing six months and another $500 upon successfully completing one year at Upper Valley Services.

Upper Valley Services (UVS) is one of Vermonts designated non-profit agencies serving people with developmental disabilities and adults who are eligible for Adult Family Care.

We are seeking a After Hours Support Responderto join our Moretown Team. Provide personalized stabilization supports to diverse individuals. Supports may include emotional regulation, positive behavior supports, personal care, and support to engage in home, recreational, and community activities.

Work alongside the After-Hours Team to provide support services for individuals with developmental disabilities and mental illness. This position requires an in-person response to assist with de-escalation, medication administration, residential staffing or accessing emergency medical and mental health services. Individual support services are intended to provide in the moment support in coordination with the After-Hours Support Line and Moretown Service Coordination Team. This position is mobile throughout Washington County and coordinates with multiple providers both in and outside of Upper Valley Services - Moretown to best meet individual or family needs.

Essential Functions:

Provide outreach or staffing support for individuals served by the Upper Valley Services Moretown team

Perform all duties as required, including but not limited to personal hygiene, meal preparation and/or medication administration

Ensure the safety and well-being of all individuals served as identified in the Emergency Fact Sheet (EFS) and Individual Support Agreement (ISA)

Monitor medical and dietary needs as appropriate

Address individual needs in a personalized manner

Transport clients in personal vehicle when appropriate

Provide coverage for staff shifts as needed for call outs

Complete other duties as assigned

Education, Knowledge, Skills and Abilities:

Bachelor's degree in related field, preferred

Two years of experience working in human services field

Experience with outreach, care coordination, crisis intervention and safety planning

Ability to work independently, communicate, and respond to stressful or emergent situations

Ability to understand the need for and maintain consumer confidentiality

Excellent written, oral, and listening skills

A valid drivers license, access to reliable transportation, insurance meeting the Agencys standards

Structure:

On Call Evenings, weekends, holidays and/or overnights

$50.00 per shift and $23.00 per hour for in-person response
