Requirements / Qualifications

Certificate Copy (Valid School Bus Driver Certificate ***Must be certified to drive a Type I bus*** front and back or sufficient training and experience as evidenced on T-01 Card)

DMV Printout (Official DMV printout no older than 90 days (H-6) or sufficient training and experience as evidenced on T-01 Card)

Driver's License Copy

Other (Valid Medical Card (front and back))
