Case Management - (Hybrid) Kennebec County

Morrison Center is looking for Case Managers to provide Adult Case Management services in the Oakland, Maine area, in response to our expanding program. This is a Hybrid rote, requiring traveling for home and community visits.

Scope of Responsibilities:

Assist clients in achieving their goals by completing necessary tasks and requirements.

Maintain documentation in compliance with regulatory agencies, such as the Department of Health and Human Services.

Conduct monthly home visits.

Develop and manage Person-Centered Plans (PCPs), assessments, and goals.

Maintain effective communication and perform research as needed. Qualifications:

Minimum of 1 year of experience working with children or adults.

Bachelor's degree in any field.

Experience with families and familiarity with PCPs and the Evergreen system is preferred. Contract Details:

Duration: This is a rolling contract that will be reviewed annually.

Notice Period: Thirty days are required for termination of the contract.

Hours: Flexible.
