The incumbent is responsible for the provision of case management services to individuals who have intellectual disabilities and/or mental illness and who reside in state training centers and hospitals, in agency approved adult homes and group homes, at home, and in various residential settings within agency catchment area. He/she is responsible for adherence to DBHDS licensing standards as well as Medicaid standards as they relate to the provision of case management services. He/she is directly responsible to the Case Management Director (ID/DD Services). Staff shall be hired with the goal of becoming welcoming, recovery-oriented, and co-occurring competent to engage all individuals receiving services in empathetic, hopeful, integrated relationships to help them address multiple issues successfully and shall support agency efforts to accomplish this. OSHA regulations apply to this position. The position is designated as a Level Two with regard to the HIPAA Privacy Rule. The position is exempt for FLSA.
Experience:
Case managers employed or contracted by the Board shall meet the knowledge, skills, and abilities qualifications in the Case Management Licensing Regulations, 12 VAC 35-105-1250. (see below). Incumbent must meet the level of education and experience required to meet Medicaid requirements for providers of Case Management Services. Meeting Qualified Mental Health Professional (QMHP) designations is preferred. A bachelor's degree in a related human services field is required. A valid Virginia driver's license is required. A flexible work schedule may be required to meet the needs of the agency/programs. Travel throughout the catchment area may be required. Medical examinations including the utilization of medical tests may be required of the incumbent.
JOB TASKS:
Responsible for case management services to individuals with intellectual disabilities (mental retardation) and/or mental illness including the acquisition of appropriate community services.
Responsible for advocacy with families, home providers, health care professionals, and other service agency personnel as needed. This includes assisting the individual or family members to apply for benefits for which they are eligible.
Responsible for case coordination for all assigned cases to include accountability for individual records, attendance at medication reviews, and the development, implementation, and monitoring of DBHDS treatment/ service plans.
Responsible for the maintenance of assigned individual electronic as needed, paper records per agency, DBHDS, Medicaid, and guidelines of other regulatory agencies.
Responsible for utilizing assessment tools to determine an individual's level of support and on-going needs
Responsible for documentation and submission of service hours/days for billing and statistical purposes.
Responsible for the enhancement of community integration by developing increased opportunities for community access and involvement, including community living skills and vocational, civic, and recreational services.
Responsible for supportive counseling to individuals, families and groups to promote implementation of services plan and community adjustment,
Responsible for making appropriate referrals to agency programs, including residential, day support and clinical.
Responsible for coordination of individual transportation, including provision of individual transportation when necessary.
Responsible for discharge planning and coordination of services for assigned cases.
Responsible for facilitating individual placement into agency approved adult homes and group homes.
