The Regional Patient Navigator is a highly organized and passionate team member who will float across a designated region, supporting patients throughout their wound care journey from initial treatment through discharge and transition to home care. This non-clinical role is instrumental in ensuring continuity of care, patient satisfaction, and coordination among care teams and facilities.
This position will not provide direct patient care but will work closely with patients, family members, and facility resources to facilitate best-practice care, resolve barriers, and maintain clear communication across the continuum of services. The ideal candidate for this role should have a strong understanding of medical terminology and familiarity with wound treatment processes, exceptional interpersonal skills, and a commitment to patient care.
Key Responsibilities
Essential functions:
· Assisting patients in understanding their post-discharge advanced wound care options
· Coordinating appointments with administrative and clinical team members
· Ensuring patients have access to wound treatments/services needed
· Build relationships and rapport with facility personnel to best support patient continuity of care process
· Facilitating resources for patients to prepare for discharge to advanced care team
· Gather the most current patient information to best support their journey
· Educating patients about the resources and support services available to them
· Acting as a liaison between patients, their families, and healthcare professionals
· Following up with patients to monitor their condition and adherence to the treatment plan provided
· Collaborating with a multi-disciplinary team to streamline comprehensive care
· Ensuring appropriate handoff to the advanced home care team
· Performs other job duties, responsibilities, and tasks at managers discretion
Skills, Knowledge and Expertise

Proficiency in electronic medical records (EMR) software is a plus
Education:
Proven work experience as a Patient Navigator, Healthcare Social Worker, or similar role
Knowledge of medical terminology and healthcare systems
Bachelor’s degree in Social Work, Nursing, Psychology or related field is desirable
