Home Health Links is seeking compassionate Physical Therapists to provide Start of Care (SOC) coverage throughout Los Angeles and Ventura County. Training and Shadowing provided.

Responsibilities:

Assess the therapeutic and rehabilitative status of the patient

Develop an appropriate treatment plan for each patient; update and modify treatment plan on an ongoing basis.

Ensure high-quality patient care

Use clinical reasoning in treating and planning, implementing, and monitoring patient progress

Maintain records with accuracy and discretionQualifications

Complete Start of Care (SOC) assessments for new patients

Degree from an accredited Physical Therapy program.

Current and unrestricted CA PT license; or ability to apply

Excellent Communication Skills

Home Health experience preferred; not required

Additional Information

We are looking for PT SOC coverage in the following areas: Ventura, Oxnard, Pasadena, Glendale, Burbank, San Fernando Valley, Monterey Park, Oak Park, Calabasas, Agoura Hills, San Gabriel Valley, West Covina, Walnut, Montrose, Duarte, Alhambra, and Hacienda Heights.
