Our Transition Youth and Young Adult Services team at Southeast Healthcare Services is seeking a dedicated and qualified professional to provide community-based services to youth ages 14-18.
This role is focused on supporting participants as they develop and strengthen independent living skills and employment readiness.
Key Responsibilities: * Conduct initial and ongoing assessments of participants' independent living skills, vocational strengths, and interests * Develop individualized service plans aimed at achieving participants' goals in areas such as education, employment, and daily living * Provide hands-on support and coaching to assist youth in obtaining and maintaining employment * Facilitate access to resources and create learning opportunities that promote self-sufficiency * Collaborate with service providers, families, and systems of care to ensure coordinated support Join our team and make a difference in the lives of youth preparing for adulthood.
Salary range based on education and related work experience = $35,000 - $42,660.
