Our campus includes a medical clinic in Hallock and a satellite clinic in Karlstad, MN. Job Summary:
A Registered Nurse (RN) in long term care is responsible for providing comprehensive nursing care to patients in a long-term care setting. This role involves assessing patient needs, developing care plans, administering medications, and collaborating with healthcare teams to ensure optimal patient outcomes. The registered nurse will also educate patients and their families about health management and support them in daily living activities.
Primary Responsibilities Include:
Supervise and provide directions of Licensed Practical Nuse (LPN), Certified Med Aide (CMA), and Certified Nursing Assistants (CNA)
Monitor and assess resident health needs and follow care plans
Administer medications and manage treatments
Provide wound care and follow infection control practices
Collaborate with healthcare teams to ensure optimal outcomes
Document nursing care, observations, and resident responses accurately and timely in EHR
Maintain compliance with all state and federal regulations, including those related to infection control, resident rights, and safety standards
Promote positivity, compassionate environment that support resident dignity and independence
Job Requirements:
Knowledge, Skills, and Abilities:
Proficiently be able to read, write, speak, and understand the English language
Strong problem-solving and critical thinking abilities
Strong verbal and written communication skills to interact with both patients and healthcare professionals
Experienced in patient assessment and care planning
Proficiency in medication administration and management
In-depth knowledge of body systems and functions to assess and manage patient health effectively
Knowledge of chronic disease management
Competence in wound care management and infection control practices
Knowledge of patient mobility assistance
Ability to educate patients on health maintenance
Strong communication with healthcare teams
High level of accuracy data entry; effective communication, and quality care
Comprehension and delivery of patient safety and healthcare ethics
Understanding of regulatory compliance and standards in long-term care
Qualifications:
Graduate of an accredited nursing program leading to licensure as a Registered Nurse (RN)
Valid MN RN license
Valid BLS certification
Experience in Long Term Care setting
Must pass MN background check and fingerprinting
Complete TB testing within the first 3 weeks of employment
Work Schedule:
Shift options: 6:00 am – 2:30 pm or 2:00 pm – 10:30 pm or 10:00 pm – 6:00 am
Every other weekend and holiday rotation required
