NOW HIRING: Remote Social Work Case Manager (Must be an OH Resident)

Pay Rate: $40.96/hour

Schedule: Monday – Friday, 8:00 AM – 5:00 PM EST

Location: Work From Home (Must reside in Ohio; minimal in-state travel may be required later in the year)

About the Role:

We are looking for an experienced Remote Social Worker Case Manager to join our team. In this role, you will provide healthcare management services to ensure patients receive the right care, improve program efficiency, and help manage healthcare costs.

What You’ll Do:

Review and determine medical necessity and appropriateness of care

Facilitate care plans to support the best possible patient outcomes

Monitor quality and compliance standards, identifying areas for improvement

Educate and empower patients to understand and follow their care plans

Promote patient advocacy and overall satisfaction

Support colleagues and help guide decision-making across teams

Implement and evaluate healthcare policies and procedures

What We’re Looking For:

Master’s Degree in Social Work (required)

Minimum 5 years of relevant healthcare or case management experience

Strong communication and problem-solving skills

Ability to work independently in a remote setting
