Job Description

Responsibilities:

Provide direct nursing care to residents under the supervision of an RN and/or physician.

Administer medications and treatments as prescribed, ensuring accurate documentation in resident charts.

Monitor residents’ health status, report changes to the supervising nurse/physician, and carry out follow-up care.

Assist with resident admissions, transfers, and discharges, including documentation and assessments within scope.

Support residents with daily living activities while promoting dignity and independence.

Maintain compliance with state, federal, and facility policies regarding infection control, safety, and resident care.

Communicate effectively with residents, families, and interdisciplinary care team members.

Participate in care conferences and contribute to individualized care planning.

Provide guidance and direction to CNAs to ensure quality resident care.

Respond to emergencies and perform life-saving interventions within LPN scope of practice.
