Definition

The selected employee will provide screenings, support, and referrals to consumers, caregivers, and agency staff through community resources, training, education, and collaboration with other professionals. The selected employee will also manage the Trualta online caregiver platform, which includes onboarding new caregivers, promoting the platform with potential community referral sources, training new staff members, updating events and weekend reads, and monitoring overall growth of caregivers accessing the platform.

This position provides oversight and technical guidance to community partners implementing the National Family Caregiver Support Program and participates in annual provider monitoring visits of all NFCSP partners in the eight-county region.

This role will facilitate Dementia support groups.

Under direction, is responsible for providing education, guidance, and/or advocacy regarding programs available through the Central Ohio Area Agency on Aging (COAAA); performs related duties as required.

Examples of Work

Coordinates a variety of community outreach efforts;

Participates in various advisory boards, task forces, councils, and committees;

Coordinates, plans, and implements special events of the Central Ohio Area Agency on Aging;

Provides comprehensive information and assistance services to consumers and caregivers;

Acts as an advocate, identifying resources, and service options available to caregivers;

Provides training on specific aging and other related topics to community professionals and agency staff;

Develops training and informational materials for workshops and presentations;

Identifies internal staff and aging network professional education needs;

Researches, reviews, and identifies materials pertinent to agency’s in-house resource center;

Maintains agency’s in-house resource center;

Coordinates professional trainings and workshops for community professionals and agency staff;

May recruit individuals to serve as volunteers; may provide advocacy to volunteers through training, support, and guidance.

Minimum Qualifications

Possession of a bachelor's degree and two (2) years of experience developing, coordinating, or conducting educational training or providing outreach services.

Substitution(s): A master's degree in nursing, geriatrics, social work, health services administration, or education may be substituted for one (1) year of the required experience.

OR

One (1) year of experience as an Aging Programs Care Coordinator, or comparable experience.

Some positions may require the possession of a valid driver’s license. Certain positions allocated to this classification may require the possession of a valid State of Ohio license as a Social Worker, depending on the nature of the position.
