Healthonomic Primary Care is seeking a dedicated and mission-driven Primary Care Physician (MD or DO) to join our full-time outpatient team. Passionate about patient-centered care and thrive in a collaborative team? Join us and make a meaningful impact every day.

Provide comprehensive outpatient care for a diverse patient population
Diagnose, manage, and treat acute and chronic conditions
Participate in quality improvement programs and internal audits
Ensure accurate CPT coding and compliance with clinical standards
Maintain timely documentation aligned with HIPAA and internal policies
Complete CME activities and maintain all licensure and certifications
Requirements
M.D. or D.O. with board certification (or eligibility) in Internal Medicine or Family Medicine
Active, unrestricted Illinois medical license and DEA registration
At least 2 years of clinical experience preferred
Credentialed or eligible for credentialing with Advocate Physician Partners and affiliated hospitals
Eligible to participate in Medicare, Medicaid, and commercial insurance panels
Strong commitment to community-focused care, collaboration, and excellence
