Health Center Case Manager
Full Time Days * $25 per hour
Lutheran Social Services of Central Ohio is currently seeking a Case Manager for our Health Center at Faith Mission in Columbus, Ohio.
Faith Mission is one of the largest homeless shelters in Columbus, Ohio, providing meals and shelter to men, women and Veterans with dignity and compassion.

What will I do as the Health Center Case Manager with Lutheran Social Services?
Our Health Center Case Manager promotes health and wellness for patients of the Health Center at Faith Mission in an effort to improve health behaviors, prevent development of chronic illness, and assist in increasing positive outcomes surrounding management of chronic illness. Works and Coordinates healthcare among an interdisciplinary team and provides services and referrals based on patient need and treatment plans. Works with patients to improve environmental factors related to health, including housing, income, benefits, health insurance coverage, food and nutrition, physical activity, substance use, and other health behaviors. Facilitates referrals and coordination of care for patients to specialty providers and community resources. Works with Faith Mission program staff members to coordinate shelter, housing, employment, and other services.
Works with patients referred by Health Center providers to improve health and wellness.
Supports treatment plan and orders from physicians and other licensed independent providers for patients.
Conduct initial pre-screening of clients to determine eligibility for services and appropriateness of case management services.
Verify enrollment in medical coverage, and support enrollment of the uninsured in Basic Health Care; including enrollment in patient assistance and other programs to assist with medication costs.
Schedules patients for medical, dental, vision, and other services available in the Health Center based on treatment plan recommendations.
Coordinating information and care requirements with other care providers.
Assist in resolving issues that could affect smooth care progression.
Coordinate and follow-up on referrals to specialty or outsides service providers.
Facilitates health and wellness education groups and activities.
Refer and link clients to appropriate services within the system of care that promote positive health outcomes, treatment adherence, and greater self-sufficiency. Monitor the client's follow-through with these services.
Provide access to emergency financial assistance (e.g. food vouchers, utility payment assistance, and transportation vouchers) as needed to promote and maintain positive health outcomes.
Links patients to Faith Mission programs, specialty care, and community resources.
Coordinates shelter, housing, employment, and resources at Faith Mission and with other systems of care, including Navigators, job programs, and resource providers.
Links patients with specialty care, agencies and community resources as needed, and coordinates treatment and support services.
Arranges and provides transportation for medically necessary services and services related to obtaining housing, employment, and benefits.
Helps patients transition to community settings, providing linkage to resources in the community and sharing information about neighborhood activities.
Documents contacts with patients in the electronic health record and on paper as needed.
Participates in quality improvement and case review activities, working with colleagues to improve quality of care and client outcomes.
Contributes to an atmosphere of safety, sanctuary, dignity, and support through adherence to the Faith Mission Code of Ethics and Confidentiality Agreements.
Assists with crisis intervention and de-escalation as needed by providing leadership, and by modeling professional and calm problem-solving behavior for clients, colleagues and the community.
Requirements for the Health Center Case Manager with Lutheran Social Services:
Bachelor's degree preferred; Minimum of Associate degree in human service field required
LSW preferred; Community Health Worker certification helpful.
Experience working with healthcare patients, people who experience homelessness, and people who experience low income preferred.
One year work experience in a social service setting preferred.
Knowledge of health and wellness concepts.
Cultural competence and skill in working with diverse populations.
Dedication to excellent customer service, dignity and respect.
Strong oral and written skills, attention to detail and strong time-management skills.
Commitment to and aptitude for working in a team environment.
Valid Ohio driver's license, access to reliable insured transportation and ability to meet LSS insurance standards required
