Licensed Practical Nurse (LPN) – Join Our Care Team
We are seeking a dedicated LPN to join our nursing team, including a PMHNP, Lead RN, and fellow LPN, to support client health and wellness.
Key Responsibilities:
Conduct basic health assessments (vitals, blood pressure, weight) and ongoing health monitoring.
Assist with medication management, process physician orders, and track client adherence.
Make referrals and follow-up for medical, psychiatric, and substance use care.
Collaborate with providers and participate in interdisciplinary team meetings.
Develop and deliver health education for groups and individuals.
Respond to medical emergencies, provide first aid/CPR, and administer Narcan as needed.
Ensure compliance with regulatory and agency guidelines.
