Job Description
Gardner Health Services is currently recruiting to fill a Clinical Data Project Manager position. This is a full-time (40 hour/week) position based out of the St. James Health Center located in San Jose, California.
The responsibility of the Clinical Data Project Manager is provide project oversight from planning to implementation to documentation of data/quality related project that supports Gardner Health Services (GHS) pursuit of clinical excellence. The main projects of this position include supporting internal preparations for Alternative Payment Models (APM), health plan incentive programs, provider empanelment, member assigned outreach, Patient Centered Medical Home (PCMH) recognition and various other timely projects per grant or regulatory requirements. This position will assist in creating, maintaining and supporting the reports essential to the smooth functioning of GHS Primary Care Division including running regularly reoccurring reports such as productivity, clinical quality measures, and reports needed for our consortium and other grant requirements. Another function of this role is the responsibility to transform Health Plans Data and coordinate provider panels/member lists to ensure that all patients are appropriately assigned to an active Primary Care Provider. This individual will also support the administrative needs of the data team, assist the CQI Director by serving as the lead in documentation preparation and submission of Patient Center Medical Home (PCMH) criteria.
Minimum Qualifications:
The minimum qualifications of the position are listed below. An equivalent combination of education and experience may be considered.
Bachelor’s Degree in Computer Science, Information Management, Health Information, Public Health, Business or related field required with at least 1 year of experience.
Minimum one year working with computer systems related to health care;
Minimum one year of data analysis experience; focus on health data preferred;
Project Management Professional (PMP) certification strongly desired.
Proficiency in FQHC related data reporting, specifically UDS, HEDIS, APM, MU, PCMH, strongly desired;
Proficiency in Excel particularly advanced level;
Proficiency in other Microsoft Office applications such as Word and PowerPoint;
Proficiency in SQL desirable but not required;
Proficiency in other data reporting and development tools such as Tableau, i2i, R, SAS, Python desired but not required.
Skills necessary to obtain EHR -certified trainer-level knowledge and complete understanding of the latest version of the EHR application;
12 month experience in health care, preferably in an ambulatory setting.
Graduate of Post-Secondary training in allied health preferred.
Knowledge of Health Insurance and/or Health Plans.
Knowledge of disease complexities.
Knowledge of medical disciplines.
Ability to interact effectively and courteously with patients and staff.
Must have legible handwriting.
Must be able to work in a fast-paced environment.

Compensation:
Compensation and level of position placement will be determined based on relevant experience and education.
Patient Care Coordinator: $90,000 - $110,000/annually
