Axiom Care is a fast-paced, rapidly growing substance abuse treatment and sober living housing provider with locations in the Phoenix metro area and Apache Junction. We primarily serve low-income and justice-involved individuals who are seeking to change their life for the better.

Job Summary

The Axiom Care OP program includes temporary housing, recovery education, employment assistance services, and high needs case management in a sober living environment with specialized clinical programming. This program serves the justice involved and SMI population who are looking to focus on recovery, build skills that enable successful reintegration into the community, reduce criminogenic thinking and behaviors and achieve independent living. The outpatient Specialty Case Managers focus on the various social determinants of health and structural obstacles that are necessary to address for the clients/residents to have the highest chance of successful recovery and community reintegration. Additionally, they assist in on-site activities and functions of the program, as assigned by the Clinical Director, Facilities Manager and/or other administrative staff. Case manager is an integral role in Axiom Care team.

Duties/Responsibilities

Maintain strict adherence to state and local ethical standards and legal requirements at all times

Complete case management functions focused on a housing first and high needs case management model

Assist clients with AHCCS applications, employment specialty services and other paperwork to aid with financial stability, autonomy and connecting with needed services

Develop client-driven individualized treatment plans and continually monitor and record treatment progress

Carry a caseload of about 20-30 clients through their residential stay

Complete high-quality documentation of any service provided, follow up needed, or other clinically pertinent information within 24 hours

Take initiative to be aware and stay updated about community resources

Assess client progress, needs and barriers in each interaction and document

Make referrals to and coordinate care with external resources regularly for all client needs

Secure client information per HIPAA standards

Assist with providing and attends training as requested

Attend clinical meetings, oversight and interdisciplinary team meetings as requested

Collaborate with the interdisciplinary team throughout the day including tech staff and leadership to ensure the best client care and open, frequent communication

Complete case management functions as needed

Assist in holding clients accountable for attendance and compliance with program expectations

Identify and report ongoing maintenance and safety issues at facility

Intervene with de-escalation, empathy and, as needed, emergency response protocol to crisis and emergency situations

Perform other related tasks and projects as assigned by management Requirements

Required Skills/Abilities

Must be proficient in computer applications and electronic medical record systems.

Proficient with Microsoft Office Suite and related software.

Ability to communicate effectively with clients, client supports and professionals.

Ability to make sound decisions in emergency situations.

Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public

Ability to define problems, collect data, establish facts, and draw valid conclusions

Working knowledge of twelve step programs of recovery

Strict adherence to ethical treatment standards

Strong time management skills

Ability to type at least 45 wpm. Education and Experience

Minimum one year of case management knowledge and experience with the AHCCCS population, justice involved persons, chronically underhoused and the substance use treatment.

One year of experience with the Crisis/SMI population strongly preferred.
