About the Role:

The Hospice Clinical Manager (RN) for the South/Central Home Team plays a pivotal role in overseeing and coordinating high-quality hospice care for patients in the South & Central markets. This position is responsible for leading a multidisciplinary clinical team to ensure compassionate, patient-centered care that aligns with regulatory standards and organizational goals. The manager will develop and implement clinical protocols, monitor patient outcomes, and provide ongoing education and support to staff to enhance clinical competencies. Additionally, this role involves collaborating closely with patients, families, and healthcare providers to facilitate effective communication and care planning. Ultimately, the Hospice Clinical Manager ensures that all clinical operations run smoothly, promoting a supportive environment that respects the dignity and wishes of patients at the end of life.

Minimum Qualifications:

Current and valid Registered Nurse (RN) license in the state of practice.

Minimum of 3 years of clinical nursing experience, ideally in home care, hospice or palliative care.

Demonstrated leadership or supervisory experience in a healthcare setting preferred.

Strong knowledge of hospice care regulations, standards, and best practices.

Proficiency in electronic medical records (EMR) and clinical documentation.

Preferred Qualifications:

Bachelor of Science in Nursing (BSN) or higher degree.

Certification in Hospice and Palliative Nursing (CHPN) or related specialty certification.

Experience managing interdisciplinary teams in a home care or hospice environment.

Familiarity with quality improvement methodologies and data analysis.

Excellent communication and conflict resolution skills.

Responsibilities:

Supervise and mentor hospice nursing staff and interdisciplinary team members to deliver exceptional patient care.

Develop, implement, and monitor clinical policies and procedures to ensure compliance with hospice regulations and standards.

Conduct regular patient care reviews and quality assurance audits to maintain high standards of clinical practice.

Coordinate with physicians, social workers, chaplains, and other healthcare professionals to create comprehensive care plans.

Provide direct clinical support and guidance to staff in complex patient care situations.

Manage staffing schedules and resource allocation to meet patient care needs effectively.

Lead staff training sessions and continuing education initiatives to promote professional development.

Serve as a liaison between the hospice organization, patients, families, and external healthcare providers.

Ensure accurate and timely documentation of patient care activities and clinical outcomes.

Participate in strategic planning and quality improvement projects to enhance hospice services.
