LOCATION: Woodrow Wilson Keeble Memorial Health Care Center Indian Health Service 100 Lake Traverse Drive Sisseton, SD 57262 CONTRACT TERM: • Period of Performance: Begins on date of award o Base Period: 6 months o Up to Nine (9) optional 6-month extensions SCHEDULE & HOURS: • Work Week: Monday – Friday • Daily Shift: Typically 8 hours between 8:00 AM – 8:00 PM o Usual shift: 11:00 AM – 8:00 PM • Weekend Hours: 4-hour shifts, 1:00 PM – 5:00 PM • Holiday Coverage: May be required; alternative weekday off may be scheduled • Expected Weekly Hours: 40–50 hours PROVIDER REQUIREMENTS: • Experience: o Minimum 2 years recent (within past 36 months) Family Medicine/General Medicine experience • Education & Certification: o Board Certified, Board Eligible, or partial completion of an accredited residency o Current BLS certification • Computer Skills: o Proficient with Electronic Health Records • Language: o Must speak, read, and write fluent English • Health & Immunizations: o No physical limitations preventing clinical work o Proof of immunizations required: Hep B, MMR, Varicella, Tdap, Flu, and PPD within past year CLINICAL DUTIES INCLUDE: 1.

Provide primary care for all ages; average of 16 patients/day 2.

Make appropriate referrals and follow through with recommended therapy 3.

Participate in the Patient-Centered Medical Home (PCMH) model 4.

Educate patients on conditions, treatments, and wellness 5.

Gather and document relevant patient history and records 6.

Deliver culturally competent care to Native American patients 7.

Ensure compliance with IHS, CMS, and facility standards 8.

Participate in assigned committees (QA, Pharmacy & Therapeutics, etc.) 9.

Engage in ongoing training and staff development 10.
