OVERVIEW
SUMMARY OF JOB
Coordinates all aspects of care and services for short term rehab patients beginning with admission process to discharge, ensuring each patient’s’ strengths and abilities are maximized. Provides excellent customer service in which patients are educated and informed of all of their options during their stay.

RECRUITMENT REQUIREMENTS
Bachelor’s degree in Social Work, Human Services or Human Services related field
Excellent communication skills and computer skills
Previous experience in the admissions/discharge for long term care preferred
Knowledge of community resources
