Monday â Friday 8:00 AM â 5:00 PM EST
About the Role:
We are seeking a dedicated Nurse Case Manager II to join our care team and make a meaningful impact on patients' lives across the Cincinnati area. In this role, youâ ll conduct comprehensive assessments of membersâ needs and determine appropriate care coordination strategies that align with their benefit plans and available services.
Key Responsibilities:
Perform comprehensive assessments of referred members to determine needs, eligibility, and care solutions.
Apply standardized case management plans, benefit criteria, and company policies to guide case resolution.
Leverage internal/external programs and services to support patient care.
Ensure compliance with regulatory and accreditation guidelines, using case and quality management best practices.
Evaluate and monitor care plans to ensure the delivery of quality, cost-effective care.
Requirements:
Experience:
Minimum 3 years of clinical experience in hospital, home health, ambulatory care, or similar settings.
Healthcare and/or managed care industry experience required.
Case Management experience strongly preferred.
Education & Licensure:
Active, unrestricted RN license in the state of Ohio.
Case Management Certification (CCM) preferred.
