Licensed Social Worker / Licensed Professional Counselor Duties and Responsibilities:
Under the supervision of the COO, Clinical Director and Program Director, the LSW/LPC will be responsible for:
Clinical services associated with the intake, monitoring, and ongoing services for clients.
Ensuring quality of care for all behavioral health participants, including but not limited to Diagnostic Assessments, Treatment Planning, Ongoing Therapy, Day Treatment, Partial Hospitalization and other Group/Individual Services.
Providing individual and group psychotherapy services to children, adolescents, and adults.
As a member of a multidisciplinary team, the LSW will be responsible for the following:
Determining the nature of a client’s situation by assessing medical, psychological, emotional, and social information through psychosocial/diagnostic assessments.
Developing treatment plans based on assessment results and engaging in on-site visits for ongoing outpatient services.
Establishing the course of action by exploring and reviewing treatment and goals with client. Update treatment plan, goals, objectives, and interventions for clients on an as needed basis.
Obtaining assistance for client(s) by referring him/her to community resources, arranging appointments and establishing rapport with other agencies if required. Makes appropriate referrals for clients, including psychiatric (within or outside of agency). Obtains client records when needed (medical, MHMRA, etc.)
Maintaining client records and progress notes through documentation of client interactions in the standard 48-hour documentation requirement.
Maintaining client confidence and protecting operations by keeping information confidential.
Providing brief intervention and crisis intervention as needed.
Conducting appropriate discharge of patients.
Engaging in community outreach and provides community presentations when needed.
Participating in case staffing and administrative meetings.
Participating in clinical supervision and seeks peer consultation as needed.
Maintaining productivity of 6 hours of patient services per day for minimum full-time employment.
