Agawam Rehab & Nursing Campus is a skilled nursing facility seeking an experienced Admissions Coordinator to fill a full-time position. As the Admissions Coordinator, you will act as a liaison to the nursing unit to help ensure a smooth transition with the admissions process for all residents.

The Admissions Coordinator will assist with the admissions process efficiently to ensure a positive experience for residents, families, and supporting staff.
Respond to all inquiries regarding potential residents of the facility in a sensitive, courteous, and competent manner.
Obtain residents' insurance information, when possible, at the time a referral is called in.
At the time of admission, meet with the resident and their families to assure all necessary forms are completed.
Maintain admission guidelines by writing, updating, and recommending changes to admission criteria, policies, and procedures.
Assist with patient queries and admissions procedures.
Organize and input information to databases.
Handling phone calls and emails from prospective patients.
Don't forget...
Clinical experience in a skilled nursing facility setting preferred.
Familiarity with post-acute care, rehabilitation hospitals, sub-acute rehabilitation facilities preferred.
