The St. John's Ministries team has an opening for a Case Manager. The number of people experiencing homelessness or housing instability has risen. We are looking for an experienced and mission-driven individual to become part of our team so that together we can build rapport and inspire guests to believe they can achieve their goals.
Position Overview
The Case Manager is responsible for providing comprehensive case management services to guests utilizing any of the St. John's Ministries facilities. The primary focus of this role is to restore the dignity of our guests by assisting them in identifying and overcoming barriers that hinder their ability to thrive. The Case Manager will work closely with guests to develop individualized service plans and provide ongoing support and advocacy. Additionally, Case Managers will collaborate as a team to create cohesive and comprehensive plans, not only for individual guests but also to enhance day-to-day interactions among guests within the facilities.
Eligibility and Assessment:
Help create eligibility criteria for both entry and continued services
Assess eligibility of guests for services
Perform due diligence to ensure all other resources of support have been exhausted
Communicate guest accountability expectations for exit and re-entry for case consideration
Complete orientations for guests
In collaboration with the Director of Security, review guest behavioral incidents at our facilities and determine consequences and outcomes
Complete homeless verification forms for eligible guests
Assess and refer guests to community housing programs
Manage denial and acceptance conversations and explain reasoning
Intake worker / Shelter position
Field community inquiries about shelter eligibility
Individual Case Management:
Establish trust and rapport with guests and assess needed support and care
Maintain awareness of available community services
Establish care management plans with guests
Serve as an advocate and help guests secure community services like mental health and transportation
Facilitate job readiness services like resume writing, job searching, and interviewing
Ensure guests have access to and consideration for Coordinated Entry
Facilitate guests in filling out applications needed for services
Assist guests in creating, maintaining, and following personal budgets as needed
Facilitate secure housing for guests, both temporary and long-term
Create and maintain landlord relationships to advocate for guest housing security
Provide case management services and aftercare after housing is secured
Schedule and complete ongoing individual case management meetings
Field and discuss concerns about St. John's Ministries' experiences
Ensure that individual guest services are comprehensive and appropriate
Case Administration:
Collaborate with the case management team and shelter operations leader to establish intake elements
Complete intakes
Enter guest data required for compliance and access to state and community services (e.g., Clarity, CaseWorthy)
Track, evaluate, and review guest notes
Survey monthly guest counts and Point In Time (PIT) and record data in Clarity
Ensure that St. John's Ministries' menu of services is comprehensive and aligned with our mission and vision
Mentor interns as assigned Skills and Abilities:
Experience working with the following issues: mental health, substance abuse, trauma, low-income and/or homeless populations, and/or healthcare preferred
Ability to act with integrity, professionalism, and confidentiality
Ability to establish and maintain professional boundaries with guests, volunteers, and co-workers
Must have a passion for the mission and population served by St. John's Ministries
Excellent verbal and written communication skills
Comfort approaching and welcoming guests with varying degrees of needs
Problem-solving skills with demonstrated ability to proactively take charge of situations and exercise appropriate judgment in guest-related situations
Ability to learn and implement use of computer programs utilized by St John's Ministries as applies to your position Qualifications and Education Requirements:
Minimum of a Bachelor's Degree in Human Services, Psychology, Human Development, Social Work, or a related field (other relevant education will be considered)
Minimum of 4 years of experience in a human services role required (other relevant experience will be considered) Work Environment
On site, in-person 1 on 1 meetings, phone calls, drop-ins, scheduled appointments, depending on guest needs
Occasional evening or weekend hours for events or urgent matters.
Operate in a professional office environment using standard office equipment.
Follow safety protocols, including the use of PPE, to minimize health risks.
Cleaning supplies are utilized to help prevent the spread of cold and flu viruses, ensuring a healthier environment for everyone. Physical Demands

Occasional standing, walking, bending, or reaching within the office environment.
