Position: Case Manager
Department: Shelter Program
Purpose/Function of Position: Servant minded leader willing to oversee the assessment and successful completion of client goals in a DV/SA/Homeless shelter. Serve as an Advocate for clients at all times. Willing to provide intake support as necessary for crisis intervention, direct care, and support services in a fast-paced environment. A keen problem-solver with the ability to find ways to say yes to clients while assisting them to success. Confident in client management solutions and consulting with supervisors through the referral process and willing to go the extra mile to provide a warm transition for all clients. Able to provide and receive constructive feedback with a goal of learning and growing. Responsible for the oversight and execution of a comprehensive and trauma informed service delivery model. Willing and able to walk alongside clients in their healing journey without fostering co-dependency. Deep understanding of case management models to provide client support, crisis intervention, and information/referral for clients and their families.
Key Responsibility Areas:
Client Services
Able to provide advocate work for ALL clients across all departments of the agency without bias
Must be able to work a flexible schedule with varied shifts including day, evening, weekend, overnight, and holiday shifts as assigned by supervisor
Willing and able to serve as a part of the on-call team
Supports immediate and timely response to all crisis calls, create a safe, warm and inviting space for crisis callers to discuss their trauma, willing to find a way to get to a yes when serving clients regardless of age, and consults with supervisor for all referred and denied services
Directs safety planning and basic family violence education to callers and shelter clients
Clearly communicates with leadership to arrange emergency transportation for incoming clients admitted to the shelter
Directs the best time and person to manage client intake process and supports the client/family through the process
Provides on-going crisis intervention and support for shelter residents
Supports the arranging of transportation for clients as needed
Conducts daily House Meeting, chore assignments, and any shelter orientation groups for new clients
Evaluates and monitors effectiveness of the client’s case management plan and modifies as appropriate to reach optimal outcomes.
Conducts classes or group sessions on a rotation with case managers designed to assist clients and families with meeting goals and/or support to successful discharge.
Regularly measures the effectiveness of client interventions to determine case management outcomes.
Maintains professional relationships with the provider community and establish a resource manual of contacts/numbers.
Maintains a professional relationship with clients, peers, and supervisors at all times.
Directs and supports case-aide in the daily support of clients for daily and weekly goals
Supports shelter staff to organize and maintain shelter supply areas and common areas
Assists shelter operations to include: adhering to shelter guidelines, policies and procedures, ensuring a calm and supportive atmosphere, assisting with the security of the shelter, managing and supporting the day-to-day needs of the clients, scheduling client service appointments, supporting client participation in group meetings with enthusiasm
Assists Shelter Coordinator with implementation of a daily schedule of activities for ALL shelter clients
Timely and accurate documentation for all client files. Exceptional communication skills log in an appropriate and timely manner. Complete all forms maintained in resident files
Completes all paperwork and reports in a timely and accurate manner; document and report duties as requested by supervisor
Regularly attends and participates in all staff meetings and case review meetings as scheduled
Prioritizes confidentiality for ALL communications regarding clients and the agency.
Grant Responsibilities
Maintain accurate case management records and database information.
Assist Shelter Coordinator with resident issues and concerns.
Develop, obtain, and implement use of case management logs and documentation.
Media/Community Relations
Actively encourage support for accomplishing the Crisis Center of Comal County’s mission, vision and values.
Uphold confidentiality requirements at all times.
Assist with fundraising activities and community events for the agency as needed
General Responsibilities
Perform other duties as assigned by the Chief of Programs
Education
Bachelor’s degree required. Will consider individuals with 10 plus years of related work experience in lieu of a bachelor’s degree. Master’s degree in social work, counseling or related field a plus.
Qualifications
Knowledge regarding dynamics of domestic violence, sexual assault, and the unhoused.
Excellent reading, speaking, writing, and computer skills.
Knowledge of resources for referring clients who are in need of additional resources.
Bilingual in English/Spanish preferred.
Must possess a personal, insured vehicle for use on the job with a valid driver’s license.
Ability to work extra hours, nights and weekends during periods of crisis.
1 – 2 years of experience as a Case Manager with other agencies is required.
Education and training may include, but are not limited to, such areas as suicide, substance addiction, sexual abuse, child abuse, domestic violence, and chemical dependency.
Background clearance check required.
