Position Summary:
The Home Health Manager oversees the day-to-day operations of the home health agency, ensuring compliance with all regulatory requirements and delivering high-quality patient care. This role is responsible for supervising clinical and administrative staff, coordinating patient care services, managing performance, and driving overall agency success.
Key Responsibilities:
Manage and supervise clinical and non-clinical staff to ensure effective and efficient patient care.
Coordinate patient admissions, case assignments, scheduling, and follow-ups.
Ensure compliance with Medicare/Medicaid regulations, state licensing requirements, and accreditation standards (e.g., Joint Commission or CHAP).
Monitor clinical documentation for accuracy and timeliness; ensure all OASIS and Plan of Care documents are complete and compliant.
Oversee and improve quality assurance and performance improvement (QAPI) programs.
Collaborate with physicians, discharge planners, and referral sources to ensure appropriate patient care and smooth transitions.
Participate in the hiring, training, and evaluation of staff.
Conduct regular team meetings, audits, and chart reviews.
Manage staffing levels and oversee patient caseloads for balance and productivity.
Participate in budget planning, cost control, and billing/reimbursement reviews.
Handle patient or family complaints and resolve service issues effectively.
Qualifications:
Bachelor's degree in Nursing, Healthcare Administration, or related field (preferred).
Minimum 3–5 years of clinical experience in home health or a related field.
At least 2 years of management or supervisory experience in healthcare.
Strong understanding of Medicare regulations and OASIS documentation.
Proficiency in EMR systems (e.g., Axxess, Homecare Homebase, HHAeXchange).
Strong leadership, communication, and organizational skills.
Valid driver’s license and reliable transportation.
Preferred Skills:
Knowledge of QA/PI processes and home health survey readiness.
Ability to interpret data and implement process improvements.
Familiarity with PDGM, HHVBP, and Conditions of Participation (CoPs).
Experience managing multi-disciplinary teams (e.g., nurses, therapists, aides).
