As an LPN, you will work closely with supervising physicians to assess, monitor, and support patients' health needs. Your responsibilities will include:

Performing client assessments and documenting observations.

Monitoring vital signs: blood pressure, temperature, heart rate, etc.

Administering medications as prescribed.

Dressing/redressing wounds and tracking healing progress.

Educating clients and families on home care techniques.

Providing palliative care to support comfort and dignity.

Recommending adaptive tools/devices to improve client quality of life.

Listening actively and addressing client concerns with empathy.Requirements

Active LPN License (in-state)

CPR Certification (American Heart Association)

Reliable Transportation for home visits

Ability to work independently and follow care plans

Friendly, compassionate demeanor

Proficiency in Microsoft Office (Word, Excel, Outlook)

Comfortable working in homes with pets (cats)

Experience in the following is a required:

Suprapubic Foley care

Ostomy bag management

Trach/Vent care

G-tube care

Pure Wick systemAvailable Shifts
