NP/PA - Rural Health Initiative - Appleton/Neenah, WI

ThedaCare is seeking an experienced Nurse Practitioner or Physician Assistant to provide comprehensive, coordinated care in Appleton and Neenah, Wisconsin.

1.0 FTE

“Best of both worlds” seeing patients in their homes & via telehealth. Company car and all technology needed is provided.

36 contact hours

4 on days in-person in patient homes. 12 on days telehealth

Registered Nurses, Community Health Workers (CHW)-will be in the patient home to facilitate the APC telehealth visits, Community Paramedic to run any rapid need interventions.

BLS/ACLS/PALS Required

ATLS Required
